- RENTAL ASSISTANCE: We assist residents in San Juan County with the first, current, or past due rent and past due mortgage. Income guidelines must be met, and assistance is not guaranteed. We understand that each situation is unique. Please call to speak with our team about available funding and assistance. At the scheduled appointment, the following documents will be requested:
- Current Lease Agreement
- First Month Rent: Proof of deposit
- Eviction or 3-day notice
- For mortgages: Mortgage statement and contract (principal payment only)
- Proof of 30 days of Income (Pay Stubs, TANF, SSI, SSDI, etc.)
- SNAP Documents
- Valid Picture ID
- CITY OF FARMINGTON PAST DUE BILLS: We assist residents in San Juan County with past-due City of Farmington Bills. Income guidelines must be met, and assistance is not guaranteed. We understand each situation is unique, please call to speak with our team about available funding and assistance.
- SAN JUAN COUNTY PAST DUE BILLS: AS funding is available, we assist in paying past-due bills with other utility providers. Income guidelines must be met, and assistance is not guaranteed. We understand each situation is unique, please call to speak with our team about available funding and assistance. At the scheduled appointment, the following documents will be requested:
- Utility Disconnect Notice
- Proof of 30 days of Income (TANF, SSI, SSDI statements)
- SNAP Documents
- Valid Picture ID
- HYGIENE, DIAPER, & INCONTINENCE KITS KITS: We assist residents in San Juan County with Hygiene, Diaper, and Incontinence Care Kits. Income guidelines must be met, and assistance is not guaranteed. We understand each situation is unique, please call to speak with our team about available Kit assistance. At the scheduled appointment, the following documents will be requested:
- Proof of San Juan County Residence
- Valid Picture ID
- Proof of Income (TANF, SSI, SSDI statements)
- SNAP Documents
- For Diaper Kits: A birth Certificate or announcement must be provided
